·         Provide your medical care

·         Access relevant Government Funding

·         In order to contact you

Besides our staff, we share this information with:

·         Other providers in order to provide your care

·         Nelson Bays Primary Health/Ministry of Health in order to obtain funding towards your care

If you choose not to provide full demographic details and ID, we will be unable to enrol you for any funded services.

 We keep your information safe by having systems in place to maintain confidentiality and to safeguard patient information and all staff sign confidentiality agreements.

 We keep your information for Ten years after last contact at which point we securely destroy it by secure disposal methods.

 You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.  What is HealthOne?

 HealthOne is a South Island based secure electronic record that allows registered healthcare providers directly involved in your healthcare, to quickly access information such as your test results, allergies, medications, GP summaries and hospital information.
